What Is a Sonographer? What They Do and How to Become One

You might meet a sonographer at a pregnancy scan, or see the word on a clinic’s team page and wonder exactly what it means.

The sonographer is the person who holds the probe, reads the images live on screen, and works out what they show.

This guide explains what a sonographer does day to day, the areas they specialise in, how the role differs from a radiographer, and how you train to become one in the UK.

What Does a Sonographer Actually Do?

A sonographer is a specialist healthcare professional trained to perform ultrasound scans. They use high-frequency sound waves to create real-time images of the inside of your body, helping to diagnose conditions and guide treatment decisions.

Ultrasound uses sound waves rather than radiation, and the images are interpreted live as the scan happens rather than captured and read later. That makes the skill and experience of the person holding the probe central to the quality of your results.

Most of a sonographer’s day is spent scanning, but the scan itself is only the middle of a longer process.

Before the scanThe sonographer reviews your referral and medical history to understand what your doctor is asking, checks any previous imaging, and explains the examination to you. That brief conversation often changes how the scan is performed. Knowing your symptoms and history helps them focus on what matters.

During the scanThey apply gel to your skin, position the probe, and adjust the machine settings to get the clearest view. They work through a set of images methodically, looking for the difference between normal and abnormal appearances.

Where Sonographers Work and What They Specialise In

Sonographers work across NHS hospitals, private clinics, community diagnostic centres and mobile scanning units. Many start as general sonographers and then focus on a particular area as they build experience.

Common specialisms include:

  • Obstetric and gynaecological: Pregnancy scans from early dating through to growth monitoring, plus scans of the ovaries and uterus.
  • Abdominal: The liver, gallbladder, kidneys, pancreas and spleen.
  • Vascular: Doppler ultrasound of arteries and veins to assess blood flow. This is used for conditions such as deep vein thrombosis.
  • Musculoskeletal: Joints, tendons, muscles and soft-tissue lumps.
  • Cardiac: Echocardiography, which images the heart. In the UK this is frequently performed by cardiac physiologists as well as sonographers.

Some sonographers work across several of these areas, while others focus on one specialism throughout their career. The training pathway is similar, with additional modules and supervised practice in the chosen area.

Sonographer, Radiographer or Radiologist: What’s the Difference?

If you’ve ever looked at a scan report or booked an ultrasound appointment, you’ve probably seen these titles and wondered what they actually mean. Sonographer, radiographer, radiologist — they sound similar, and people often use informal terms like “ultrasound tech” or “sono technician” without realising these aren’t official job titles.

Here’s the thing: they describe completely different roles with different training pathways, and knowing the difference matters when you’re trying to understand who’s scanning you and who’s interpreting your results.

In practice the lines can blur. Many sonographers trained first as radiographers before specialising in ultrasound. A radiologist may review a sonographer’s scan when a case is complex or needs a second opinion.

How to Become a Sonographer in the UK

There’s no single route into sonography in the UK, which can make the career path feel confusing at first. The unifying requirement across all pathways is a qualification accredited by the Consortium for the Accreditation of Sonographic Education (CASE), which most NHS trusts and private clinics look for when hiring.

The main routes

Usually 12–24 months

Postgraduate route (most common)

You qualify first in a related healthcare field, most often diagnostic radiography, nursing or midwifery. After gaining some clinical experience, you complete a postgraduate certificate, diploma or MSc in medical or clinical ultrasound. This remains the most common path, and most programmes take 12 to 24 months while you work in a scanning role.jobs.

Direct undergraduate route

Some universities now offer a three-year BSc in medical or clinical ultrasound, allowing entry without a prior healthcare degree. The University of Leeds, for example, runs a CASE-accredited three-year programme designed specifically for sonography.

Minimum 36 months

Degree apprenticeship

An integrated sonography apprenticeship lets you earn while you train. It takes a minimum of 36 months and combines academic study with supervised clinical practice. Some NHS trusts and universities now offer this route.

Postgraduate pathway

Conversion from another profession

Nurses and midwives can move into ultrasound through postgraduate training. Entry typically requires current NMC or HCPC registration and a relevant degree.

What the training involves

Postgraduate courses typically run for 12 to 24 months and require an agreed clinical placement, because so much of the learning happens with a probe in hand. You’ll need to be working in a scanning environment while you study, which is why many sonographers start as healthcare assistants or radiography assistants before moving into formal training.

Entry requirements vary by route. For undergraduate degrees, you’ll usually need two or three A levels (or equivalent) plus GCSEs in Maths, English and Science. For postgraduate study, you’ll need a relevant healthcare degree and professional registration with the HCPC or NMC.

How long does it take?

Overall, becoming an autonomous sonographer commonly takes between four and seven years, depending on your starting point. Most sonographers first complete a three-year undergraduate healthcare degree, then gain two to three years of clinical experience before undertaking a one to two-year postgraduate ultrasound qualification alongside supervised practice.

Professional registration

Sonography is not a statutorily protected title in the UK, but almost all employers require HCPC registration (via an underlying radiography, nursing or midwifery qualification) plus a CASE-accredited postgraduate ultrasound qualification. Many sonographers also register on the Public Voluntary Register with the Society and College of Radiographers.

Worth knowing

Sonography is not a statutorily regulated profession in the UK, which means “sonographer” is not a protected title in the way “radiographer” or “doctor” is. Reputable employers still expect a CASE-accredited qualification, and many sonographers register voluntarily with the Professional Standards Authority accredited Register of Clinical Technologists. NHS Employers notes that sonographers who are not statutorily registered cannot act as a referrer for scans that use ionising radiation.

The Parts of the Job People Underestimate

Let’s be honest about the parts of sonography that don’t make it into the glossy career guides.

  • It’s physically demanding. Holding a probe with sustained pressure, often in awkward positions, places real strain on the shoulder, neck and wrist. A 2024 systematic review in BMC Health Services Research found that work-related musculoskeletal disorders are highly prevalent among sonographers, making it one of the higher-risk healthcare groups for these injuries. Good posture, ergonomic equipment and regular breaks matter.
  • Some findings are difficult. A sonographer is sometimes the first person to see that something is wrong, including in situations as sensitive as an early pregnancy with no heartbeat. Handling those moments with calm and clarity is a core skill.
  • The scan is not the whole answer. In some settings a sonographer can talk you through what they are seeing. In others, particularly parts of the NHS, results go to the referring clinician first. Ultrasound also has limits. It identifies features that need further assessment rather than delivering a final diagnosis by itself.
