A two-week-wait breast referral, often written as 2WW or urgent suspected cancer referral, is what your GP sends when they find, or you describe, a symptom that needs specialist breast assessment fast. Under NHS standards you should be seen at a hospital breast clinic within 14 days.

At the appointment you have a clinical examination by a breast surgeon or specialist nurse, an ultrasound and, from about age 40, a mammogram, and if anything abnormal is seen a biopsy is done at the same visit. Most people who go through a 2WW referral do not turn out to have cancer. The pathway is deliberately cautious.

Ultrasound uses high-frequency sound waves rather than X-rays, and is particularly useful for looking at a specific area of concern, such as a lump, an implant or a targeted symptom, and in younger patients with denser breast tissue, where mammography is harder to read.

Mammography uses low-dose X-ray and is the standard test for population screening, especially from about age 40 to 50 upwards, because it is more sensitive to the very small calcifications that can be an early sign of cancer.

In practice, breast clinics run them together. The One-Stop Breast Clinic combines both plus a surgeon review, precisely because each test sees things the other misses. If you are under 40 and asymptomatic, ultrasound alone is often the more useful private test. Over 40, the mammogram becomes central to the answer.

The Breast Implant Ultrasound (£269) assesses implant integrity, signs of rupture (intracapsular and extracapsular), capsular changes and the surrounding breast tissue, and it is the appointment to book if you have new pain, a change in the shape or firmness of an implant, suspected leakage, or if you are due a routine post-surgical check.

Ultrasound is the usual first-line imaging test for implant assessment because it does not require compression of the breast. If additional imaging is recommended after the ultrasound, such as MRI, we will explain that in the room and set out the route.

Breast shear wave elastography adds a quantitative measurement of tissue stiffness, in kilopascals (kPa), to a conventional breast ultrasound. Stiffer tissue is more likely to be abnormal, and having a number to work with can help characterise a finding when BI-RADS category, clinical history and prior imaging leave uncertainty.

It is used as an adjunct to conventional imaging, not a replacement for it, and it does not replace biopsy where biopsy is indicated. Most people booking a routine breast ultrasound do not need it.

We do not do biopsies at either clinic ourselves. Where a straightforward same-visit surgeon review would answer the question, the One-Stop Breast Clinic often removes a second appointment altogether.

For a breast ultrasound, no preparation is needed. Wear two pieces rather than a dress so you can undress from the waist up easily, and arrive 10 minutes early. Bring any previous breast imaging, biopsy or clinic letters if you have them.

For a mammogram appointment (One-Stop Breast Clinic), avoid deodorant, talc, perfume or moisturiser on the chest and underarms on the day, as these can show on the film. If you are still having periods, the week after your period is generally the most comfortable time for a mammogram; it is not clinically essential, but it is worth noting when you book.
