Doppler vascularity can change the treatment pathway
Doppler ultrasound shows whether a tendon has active neovascularity. This helps distinguish active tendinopathy from adjacent bursitis or sheath inflammation and can influence whether PRP, corticosteroid, physiotherapy or further imaging is more appropriate.
High Doppler signal
Often supports active tendinopathy and may favour a tendon-healing strategy such as load management or PRP.
Low or absent Doppler
May suggest inactive degeneration or a different pain generator such as bursal or sheath inflammation.

Is ultrasound better than MRI for tendon pain?
