A standard 2D scan cannot show that plane directly, which is why a 3D scan of the uterus is the preferred ultrasound for checking womb shape.

It depends on which kind of 3D ultrasound you mean, because two very different scans share the name. A 3D or 4D baby scan in pregnancy is a keepsake or bonding scan of the baby’s face, offered by many private pregnancy studios, and is often priced somewhere around £80 to £200. A 3D pelvic or 3D uterus ultrasound is a diagnostic gynaecological scan of the womb and ovaries outside pregnancy, performed by an ultrasound specialist and written up as a formal report, and it costs more.

A standard 2D pelvic ultrasound shows flat, real-time slices of the uterus and ovaries, lengthways and crossways, and it is the right first test for most pelvic symptoms. A 3D pelvic ultrasound starts with the same 2D scan and then captures a volume of the uterus, which is rebuilt so it can be viewed in any plane, including the coronal plane, and measured afterwards.

That matters in three situations. It shows the outside contour of the top of the womb and the cavity together, which is what separates a septate from a bicornuate uterus. It shows exactly where fibroids and polyps sit relative to the cavity, which affects fertility and surgical planning. And it shows the arms of a coil in relation to the cavity, so a displaced or embedded device can be identified. If your question is simply whether there is a cyst or a fibroid, a 2D scan is usually enough, and we will tell you if that is the case.

Both describe a womb with a dividing indentation at the top, but in different places, and 3D ultrasound is one of the best ways to tell them apart. In a septate uterus the outside of the womb is a normal smooth shape and the division is on the inside only: a band of tissue, the septum, projects down into the cavity. In a bicornuate uterus the outside of the womb is itself dipped in the middle, so the uterus has two horns with a deep external notch between them.

On the coronal 3D view the sonographer measures both the outside contour of the fundus and the depth of the internal indentation, and classifies the shape using the ASRM or ESHRE/ESGE criteria. The distinction matters because the two are managed differently. A septum is associated with miscarriage and can sometimes be removed hysteroscopically when a specialist feels it is clinically indicated, whereas a bicornuate uterus is not usually operated on and is managed through closer monitoring in pregnancy.

A 3D scan can show the whole range of uterine shape variants. An arcuate uterus has a shallow, broad dip at the top of the cavity with a normal outside shape and is usually regarded as a normal variant. A unicornuate uterus has only one horn, giving a narrow, banana-shaped cavity, sometimes with a small rudimentary horn on the other side. A uterus didelphys is a double uterus with two separate uterine bodies and two cervices. A T-shaped uterus has a narrow cavity shaped like a T because the side walls are thickened.

A normal uterus on 3D ultrasound shows a smooth outside contour and a triangular cavity with a straight or gently convex top. Many people only discover a shape variant during fertility investigations or after a miscarriage, and many variants cause no problems at all. Your report states the shape in plain language, how it was classified, and whether referral to a gynaecologist or fertility specialist would be sensible.

Fibroids: yes, and this is one of the main reasons to choose 3D. It maps the number, size and position of fibroids and, most importantly, how far each one bulges into the cavity, which is what decides whether it is likely to affect bleeding or fertility and whether it can be removed hysteroscopically. Polyps: 3D helps show polyps in the lining and how they relate to the cavity, and is best done in the first half of the cycle when the lining is thinner.

Adenomyosis: 3D can show changes in the junctional zone, the inner layer of the womb muscle, which supports a diagnosis alongside the 2D features. Endometriosis: a pelvic scan can show endometriomas (endometriosis cysts on the ovaries) and some signs of deeper disease, but a 3D view of the uterus is not a test for endometriosis in itself, and a normal scan does not rule it out. If endometriosis is the main question, tell us when you book so we can advise on the right scan.

Yes. 3D ultrasound is very good at checking a coil, because the coronal view shows the whole device, including both arms of a T-shaped coil, in relation to the cavity. It can confirm that a coil is correctly placed at the top of the cavity, or show whether it has moved down towards the cervix, rotated, or partly embedded in the wall of the womb.

People usually book this when they cannot feel the threads, when there is new pain or a change in bleeding after fitting, or before removal if a previous attempt was difficult. The report describes the position clearly so your GP, sexual health clinic or gynaecologist can decide what to do next. A coil check can be done at any point in the cycle.

Yes, and it is one of the scans fertility specialists most commonly ask for. Before IVF or other fertility treatment, 3D ultrasound shows the shape of the cavity and whether a septum, fibroid, polyp or scarring could interfere with implantation. After two or more miscarriages, assessment of uterine shape is part of the standard work-up, and 3D ultrasound is a non-invasive way to do it.

The scan also reviews the ovaries for cysts and PCOS appearance. It does not check whether your fallopian tubes are open, which needs a different test. If fertility is the main question, our fertility ultrasound scan or fertility clinic may be a better starting point, and we are happy to advise.

The best 3D images of the womb come from a transvaginal (internal) scan, because the probe is closer to the uterus. A transvaginal scan is always optional and only performed with your consent. It is usually no more uncomfortable than a smear test, you can ask for a chaperone, and you can stop at any time. If you prefer not to have an internal scan, a transabdominal scan can still give useful information, though the 3D detail of the cavity is usually lower, and the report will say so.

For the first, transabdominal part, drink about one litre of water an hour before your appointment and try not to empty your bladder. You will be asked to empty it before the internal part. Eat and take your usual medication as normal. Bring any previous scan reports, fertility or gynaecology letters, and details of your cycle and any coil.

Timing depends on the question. For uterine shape and septate or bicornuate questions, the second half of the cycle, after ovulation, is ideal because the thicker lining outlines the cavity well.
