Muscle pain may result from acute sporting injury, direct trauma, overuse, haematoma, fascial defects or a soft tissue lump. Ultrasound helps identify the structural cause and supports a safer recovery plan.

- Muscle strain or partial tearUltrasound shows fibre disruption, oedema and haematoma, helping estimate severity and recovery time.
- Complete muscle tearA full-thickness tear can show a visible gap, retraction and haematoma. Significant complete tears may require urgent onward referral.
- Muscle contusionDirect trauma may cause a “dead leg” or intramuscular haematoma. Ultrasound measures the collection and monitors resolution.
- Delayed-onset muscle sorenessDiffuse pain after exercise can be distinguished from a focal structural tear.

Grade 1
Minor fibre disruption or oedema. Usually shorter recovery.
Grade 2
Partial tear with haematoma. Recovery depends on size and tendon involvement.
Grade 3
Extensive tear with significant haematoma and longer return-to-sport timeline.
Grade 4
Complete tear or avulsion. May require urgent surgical opinion.
