Sperm production depends on a signalling chain running from the brain to the testicles. When a semen analysis is abnormal, these six markers help locate where along that chain the problem sits.

Trying to conceive without success

One caveat worth understanding: these are reference limits drawn from men who conceived within twelve months, not a pass mark. A result above the limit does not guarantee conception, and a result below it does not rule it out. Motility is also only one of several parameters, which is why it is read alongside count, morphology, volume and vitality rather than on its own.

What is a normal sperm count?

The World Health Organization sixth-edition lower reference limits are 16 million sperm per millilitre for concentration and 39 million for the total number in the ejaculate. Below these figures the term used is oligozoospermia; where no sperm are found at all, azoospermia.

Why is a testicular ultrasound included?

Because a semen analysis measures the output without showing the cause. The scan assesses testicular volume and structure, the epididymis, and the veins draining the testicle.

A varicocele is the most common finding relevant here. It is present in around 15% of men generally and considerably more often among men investigated for infertility, it produces no symptoms in many cases, and it is treatable. It cannot be detected by a semen analysis. The scan also identifies cysts, undescended or small-volume testicles and signs of obstruction.

What does the hormone profile in the Advanced test show?

It measures FSH, luteinising hormone, testosterone, free androgen index, prolactin and sex hormone binding globulin.

Together these show whether the signal from the brain to the testicles is working normally, whether the testicles are responding, and how much usable testosterone is circulating. A raised FSH with a low sperm count points towards a problem in the testicle itself; a low FSH points towards the pituitary. Prolactin is included because a raised level can suppress the whole system and is treatable.

Are home sperm test kits accurate?

Home kits typically measure sperm concentration alone, and some only indicate whether the count is above or below a single threshold. They can be a reasonable first prompt to seek testing.

What they do not assess is motility, morphology, vitality, volume or antibodies, and a normal concentration alongside poor motility would be missed entirely. They also cannot show a varicocele or a hormonal cause.

Do you offer post-vasectomy sperm tests?

A post-vasectomy semen analysis is a different test with a different purpose: it checks whether any sperm remain, rather than assessing fertility.

Do you test sperm DNA fragmentation?

Not as part of these packages. Sperm DNA fragmentation is a specialist test, usually requested by a fertility clinic in specific situations such as recurrent miscarriage or repeated IVF failure.

What happens if my results are abnormal?
