Fertility Assessment Scan

A fertility assessment scan is a thorough gynecological Ultrasound assessment of your pelvic organs: womb, the lining of the womb, ovaries, fallopian tubes, and surrounding structures, which is usually performed in the initial stage of the cycle usually on day five and in some cases even earlier as day 2 and is usually called baseline scan. The aim of the scan is to assess the pelvis, ovaries, uterus, and the lining of the womb the endometrium. This is being done prior to the start of the treatment cycle for IVF, Embryo transfer, Donor eggs, Donor sperms, IUI, and on natural cycles without any treatment. From then we follow the progress of your natural cycle to predict the time of ovulation for natural conception. During treatment cycles, regular scans are necessary to check the response of your body to the treatment you are receiving. This will help your fertility center or your GP to adjust your treatment accordingly.
This type of scan will also rule out any structural cause that might explain difficulty conceiving, recurrent miscarriages, generalized pelvic pain, or any other related issues. We will also count how many follicles are visible at the beginning of the cycle within each ovary (antral follicular count), and comment on the accessibility of the ovaries for better planning of the upcoming IVF egg retrieval procedure.

Usually following your fertility assessment by a specialist, you are referred to have a pelvic scan. The aim of the scan is to check the female internal pelvis to check the health of the uterus, ovaries and the lining of the womb. This will provide the specialist or the fertility clinic an insight to the possible structural cause of possible fertility issues. We will assess any structural cause that may be associated with reduced fertility, such as fibroids, endometrial polyps, PCOS, abnormal shape of the womb or lining, adenomyosis, endometriosis, or reduced follicular count.
  • You will be asked a series of questions related to your menstrual period and relevant medical history
  • Perform the Ultrasound scan and take high-quality representative images
  • Explain ultrasound findings and answer your questions or concerns within our scope of practice
  • Issue a digital copy of the report within 24 hrs, containing relevant images for you to keep for your records, or take to your GP or specialist
  • Recommend a follow-up ultrasound scan if necessary
  • Offer a GP or specialist referral and a Blood Test if deemed necessary
There is a wide range of findings we may find during your Fertility Assessment Ultrasound Scan, aside for normal appearances of the pelvic organs, the most common are:
  • Reduced Follicular Count
  • Uterine fibroids (benign growth in the muscle of the womb)
  • Ovarian cysts (fluid-filled structure within the ovaries)
  • Endometrial polyps (benign growth within the lining of the womb)
  • Polycystic ovaries (suggestive of hormonal imbalance)
  • Uterine malformations (the shape of the womb may vary)
  • Hydrosalpinx (fluid within fallopian tubes)
  • Adenomyosis (condition when the cells of the lining of the womb implant within the muscle of the womb)
  • Endometriosis (condition when the cells of the lining of the womb implant within the surrounding pelvic structures)
  • Evidence of pelvic inflammatory disease
Usually, your referrer would request a series of blood tests for you at this initial stage of pre-IVF assessment, which we are happy to arrange for you.
We generally perform this scan in two parts: transabdominally (on top of the tummy) and transvaginally (internal scan), with transvaginal views giving the most details. The transabdominal part is performed on a full bladder (finish drinking 0.75-1L of water about 1 hr before your appointment time), after which you will be asked to empty your bladder before your internal scan.
The Ultrasound Specialist will explain the procedure before your scan. You will be provided with a gown to respect your modesty in the likelihood of an internal scan. It will then involve the insertion of the tip of the transvaginal probe, covered with a latex-free probe cover with a small amount of sterile gel into the vaginal canal. Our specialist will have to slightly move the probe to assess the relevant anatomy and to produce high-quality diagnostic ultrasound images. You may feel mild discomfort during your ultrasound scan; however, you can stop or pause the examination at any moment. The Pre-IVF Assessment ultrasound scan is regularly completed within 10-15 minutes. Our Sonographer will recommend the best course of action, depending on the ultrasound scan findings.
You will get the results verbally after the scan, followed by a digital copy of the report with relevant images after or within 24 hrs.
