Hamstring pain can be caused by acute muscle tears, tendon overload, proximal tendon degeneration, scar tissue, haematoma or sciatic nerve irritation. Ultrasound helps identify the specific structure involved.

Acute muscle and tendon injuries

  • Biceps femoris strain The most common hamstring injury, often occurring during sprinting, kicking or sudden acceleration. Ultrasound can identify fibre disruption, haematoma and tendon involvement.
  • Semitendinosus strain Injury may involve the muscle belly, internal raphe or proximal conjoint tendon. Ultrasound helps distinguish isolated muscle strain from tendon-associated injury.
  • Semimembranosus strain Often related to deceleration or overuse. Ultrasound checks muscle belly, tendon thickening and partial tearing.
  • Proximal hamstring avulsion Sudden severe buttock or posterior thigh pain after forceful movement may represent tendon detachment from the ischial tuberosity and requires urgent specialist assessment.

Chronic and overuse conditions
